Placement of a child (children) in a foster family
How to get the service at a government agency (required documents):
- Application on the willingness to take a child into a foster family.
- An ID document or an electronic document from the Digital Documents service (for identification).
- Notarized consent of a spouse, if married.
- Income information of the person wishing to take a child into a foster family and/or their spouse, if married, for the past 12 months.
- Medical certificates of the person wishing to take a child into a foster family and/or their spouse, if married, confirming the absence of diseases in accordance with the list approved by the Order of the Minister of Health and Social Development of the Republic of Kazakhstan as of August 28, 2015 No. 692 “On approval of the list of diseases that prevent a person from adopting a child, taking a child under guardianship or custody, or into a foster family (patronage)” (registered in the Register of State Registration of Regulatory Legal Acts under No. 12127).
- Copies of documents confirming the right to use the housing of a service recipient and (or) spouse(s) (in the absence of ownership right for housing).
- Marriage certificate (if married) or an electronic document from the Digital Documents service (required if information is not available in the “Civil Registry Office Registration Point” information system).
- A certificate of completion of training for individuals wishing to adopt orphan children or children who have been left without parental care (excluding the child's close relatives).
Result of the service delivery
Notification on the conclusion of a contract for placing a child (children) in a foster family and/or a reasoned refusal to provide the public service.
