How to get the service online
- Sign in on the Portal and click "Order Service Online" button.
- Fill in an application and sign it using a digital signature or SMS password (it is mandatory to have registration in the Mobile Citizens Database).
- In your personal account (in "History of Obtained Services" section) familiarize with a notification on application processing status, which will be sent within the specified time.
- Visit the Civil Registry Office at the time specified in the notification with the following documents:
- original copy of medical birth certificate;
- original copy of the mother's
ID card;
- original copy of the father's ID card.
How to get the service at a government agency (required documents):
For state registration of birth:
- Application on the birth.
- An ID document or an electronic document from the Digital Documents service of an applicant or representative under a notarized power of attorney (for identification).
- A copy of the parents' marriage certificate (when registering the act outside the Republic of Kazakhstan and in the absence of information in the Civil Registry Office IS).
- A notarized power of attorney, in case if an application is submitted by a service recipient`s representative.
Foreigners and stateless persons should also submit the following:
- A foreigner permanently residing in the Republic of Kazakhstan submits a residence permit for a foreigner in the Republic of Kazakhstan.
- A stateless person residing permanently in the Republic of Kazakhstan must present a certificate of a stateless person.
- A foreigner or a stateless person temporarily staying in the Republic of Kazakhstan shall present an identity document issued by the relevant authorities of the country of which the foreigner is a citizen or the state of permanent residence of the stateless person and a notarized translation of their text into Kazakh or Russian.
The correctness of translation of a foreigner/a stateless person documents can be attested in a diplomatic mission or consular institution, or in the foreign service of the state of which the foreigner is a citizen, or the state of permanent residence of a stateless person.
An application for state registration of the birth of a child over one year of age must be accompanied by a medical birth certificate (if not available in electronic form).
How to get the service at "Government for Citizens" Public Corporation (required documents):
- An application for introducing amendments, additions and corrections.
- An ID document or an electronic document from the Digital Documents service (for identification).
- The birth certificate, in case of loss, indicating its loss and information about the state registration of the civil status act.
- Copies of certificates of state registration of civil status acts issued outside the Republic of Kazakhstan, legalized or stamped with an apostille, with a notarized translation into Kazakh or Russian, confirming the need to make amendments, additions and corrections to the birth certificate.
- Information about documents confirming the need to make amendments, additions and corrections (information on civil status acts registered in the territory of the Republic of Kazakhstan, information on court decisions on amendments, corrections, additions, on the establishment of a legal fact, etc.).
- A document confirming payment for the state registration of civil status acts or a document which is the basis for exemption from payment (if required) under paragraph 2 of Article 182 of the Code.
- A notarized power of attorney, in case if an application is submitted by a service recipient`s representative.
How to get the service at a government agency (required documents):
To make changes, additions and corrections to a birth record:
- Application to establish paternity (maternity), to establish paternity at the request of a person who recognizes himself as the father of a child, to establish paternity by a court decision, to adopt a child, to make changes, additions and corrections to the form according to the Appendices 9, 10, 11, 12, 32 ( depending on the reason of the application for amendments, additions and corrections).
- An ID document or an electronic document from the Digital Documents service (for identification).
- The birth certificate, in case of loss, indicating its loss and information about the state registration of the civil status act.
- Information on documents confirming the need for amendments, additions and corrections (information on the court decision on adoption, establishing paternity; on the court decision on annulment or recognition of the invalidity of the adoption; when establishing paternity, documents confirming the circumstances of the mother's absence, depending on the reasons stated in the father's statement): When establishing paternity, documents confirming the circumstances of the mother's absence, depending on the reasons stated in the father's statement: information on the registration of the civil status act on the mother's death (in the territory of the Republic of Kazakhstan); information on the court's decision to declare the mother incompetent, to declare her deceased, to deprive or limit her parental rights; a certificate of inability to establish the mother's place of residence, information on the court's decision on amendments, corrections, additions, on the establishment of a legal fact, etc.).
- Copies of certificates of state registration of civil status acts issued outside the Republic of Kazakhstan, legalized or stamped with an apostille, with a notarized translation into Kazakh or Russian, confirming the need to make amendments, additions and corrections to the birth record.
- A document confirming payment for the state registration of civil status acts or a document which is the basis for exemption from payment (if required) under paragraph 2 of Article 182 of the Code.
- A notarized power of attorney, in case if an application is submitted by a service recipient`s representative.
If an application to establish paternity is made before the birth of the child, a medical certificate confirming the mother's pregnancy issued by a medical organization or private practitioner.
If the paternity is established at the same time as the birth is registered, a birth certificate is not required.
If amendments are made to the birth certificate relating to a change of name, patronymic or surname, an application for a change of name, patronymic or surname according to Appendix 7.
A 3*4 cm photo of the service recipient.
The following categories are exempt from paying for the service of state registration of civil status acts upon presentation of supporting documents:
1) Veterans of the Great Patriotic War, veterans of the Great Patriotic War and veterans of military operations on the territory of other states, persons who were awarded orders and medals of the former USSR for selfless work and irreproachable military service in the rear during the Great Patriotic War, persons who worked (served) for at least six months in the period from June 22, 1941 to May 9, 1945, and persons who were not awarded orders and medals of the former USSR for selfless work and irreproachable military service in the rear during the Great Patriotic War, and those who have not been awarded orders and medals of the former USSR for selfless work and irreproachable military service in the rear during the Great Patriotic War, persons with disabilities, as well as one of the parents of a person with disabilities since childhood, a child with disabilities, guardians (trustees) upon presentation of supporting documents - for the state registration of acts of civil status and the issuance of repeated certificates and certificates of state registration of acts of civil status.
2) State organizations – for issuance of duplicate birth certificates.
3) Individuals – for state registration of a child's birth and issuance of a birth certificate.
4) Individuals – for state registration of death and issuance of certificates, duplicates, statements and death notices.
5) Individuals – for the cancellation of civil status acts.
6) Individuals – for issuing certificates when amending, supplementing, restoring and correcting records of civil status acts in connection with errors made during the state registration of civil status acts.
7) Individuals – for making changes and additions to the records of the state registration of birth in connection with the establishment of paternity, adoption of a child by citizens of the Republic of Kazakhstan and the issuance of duplicate birth certificates in connection with adoption and establishment of paternity.
