How to get the service at "Government for Citizens" Public Corporation (required documents):
- An application.
- An ID document or an electronic document from the Digital Documents service (for identification).
- Notarized consent of a spouse, if married.
- Certificates confirming the health status of the recipient and their spouse (if applicable), as well as certificates confirming the absence of diseases in accordance with the List. Also the certificates confirming the absence of any information about the condition registered in drug treatment and psychiatric dispensaries, in accordance with the form approved by the Order.
- A copy of the marriage certificate (if applicable) is required if there is no information in the Civil Registry Office Information System (CRO IS) or if the marriage took place outside the Republic of Kazakhstan.
- Birth certificate of a child (children) in electronic form or its copy on paper, in the absence of information in the CRO IS or if a child was born outside the Republic of Kazakhstan (an original copy is required for identification).
- Copies of documents confirming the absence of parental custody (e.g. death certificate, court decision on deprivation or restriction of parental rights, declaration of parents as missing or incapacitated, court verdict on parents serving sentences in prisons). Documents confirming the search for the parents, the removal of the child (children) from the parents, the parents' long-term treatment in healthcare organizations, the act of abandoning the child (children), the application for abandoning the child (children) and the birth certificate (in the absence of information in the registry office), all in the form approved by the Order.
- Information about the income of a service recipient and (or) spouse(s), if married.
- Copies of documents confirming kinship of relatives, stepfather (stepmother) to a child (children)
- A certificate of completion of training for individuals wishing to adopt orphan children or children who have been left without parental care (excluding the child's close relatives).
- Copies of documents confirming the right to use the housing of a service recipient and (or) spouse(s) (in the absence of ownership right for housing).
The documents are submitted in original copies and afterwards are returned to a service recipient.
When referencing the portal:
- An application in the form of an electronic document signed by the service recipient's DS or certified with a one-time password when registering and connecting the service recipient's subscriber number provided by the mobile operator to the portal account.
- An electronic copy of the notarized consent of the spouse, if married.
- An electronic copy of the health status certificate for the recipient and their spouse (if applicable), confirming the absence of diseases in accordance with the list approved by Order No. 692. Additionally, certificates confirming the absence of any registered conditions in drug treatment and psychiatric dispensaries, in accordance with the form approved by the Order No. KR DSM-49/2020.
- If married, an electronic copy of the marriage certificate is required in the absence of information in the CRO IS or outside the Republic of Kazakhstan.
- An electronic copy of the child's (children's) birth certificate is required if there is no information in the CRO IS or if the child was born outside the Republic of Kazakhstan (the original copy is required for identification purposes).
- Electronic copies of the recipient's and/or their spouse's income documents.
- Electronic copies of documents confirming the absence of parental custody (e.g. death certificate, court decision on deprivation or restriction of parental rights, declaration of parents as missing or incapacitated, court verdict on parents serving sentences in prisons). Documents confirming the search for the parents, the removal of the child (children) from the parents, the parents' long-term treatment in healthcare organizations, the act of abandoning the child (children), the application for abandoning the child (children) and the birth certificate (in the absence of information in the CRO IS), all in the form approved by the Order No.112.
- Electronic copies of documents confirming the right to use the housing of a service recipient and (or) spouse(s) (in the absence of ownership right for housing).
- Electronic copies of documents confirming kinship of relatives, stepfather (stepmother) to a child (children).
- An electronic copy of the certificate of completion of training for individuals who wish to adopt orphaned children or children who have been left without parental care, with the exception of close relatives of the child.
The provision of a child's (children's) birth certificate and the documents specified in subparagraph 7) of the list provided to the service provider and the Public Corporation is not required if the child (children) live in organizations for orphaned children and children left without parental care.
The guardianship or custodianship authorities performing transfer orphans and children left without parental care to relatives, stepfathers (stepmothers) under guardianship or custodianship in accordance with the requirements of Clause 1 of Article 122 of the Code of the Republic of Kazakhstan "On Marriage (Matrimony) and Family".
Admission is carried out in the order of the electronic queue, at the place of registration of the service recipient, or at the place of registration of a minor in need of custody.
