How to get the service online

1. Sign in on the Portal and click "Request Online" button.
2. Fill in an application and sign it using DS (digital signature).
3. In your personal account (in "History of Obtained Services" section) familiarize with a notification on application processing status, which will be sent within the specified time.

How to get a service from a government agency or from "Government for Citizens" Public Corporation (required documents):

1. Application in the form 1 or 2.
2. An ID document or an electronic document from the Digital Documents service (for identification).
3. A court decision recognizing a citizen as legally incapacitated or partially incapacitated.
4. Marriage/divorce certificate.
5. The statement of the medical advisory commission in the 026/y form (when establishing custodianship).
6. Certificates confirming the absence of information on registration in narcological and psychiatric dispensaries.
7. Written consent to the establishment of guardianship (in any form) from all family members living with the potential guardian over the age of 18 (if the ward is expected to live with the guardian).

Information on the absence of the fact of incapacity or limited legal capacity of the recipient of the service, on the presence of an adult over whom the data on the incapacity or limited legal capacity are established, on the first head of the center for the provision of special social services (if it is an employee of the center for the provision of special social services) and on the above-mentioned paragraphs 2, 3, 4, 5, 6 are received in the form of electronic documents certified by the digital signature of authorized officials from the relevant state information systems. An electronic request for consent to establish custody from all family members over the age of 18 living with a potential guardian is sent to their personal account through the e-Government portal.

In the absence of information in the information systems, the relevant documents are attached to an application.
