How to get the service online
- Sign in on the Portal and click "Request Online" button.
- Fill in an application and sign it using a digital signature or SMS password (it is mandatory to have registration in the Mobile Citizens Database).
- In your personal account (in "History of Obtained Services" section) familiarize with a notification on application processing status, which will be sent within the specified time.
How to get the service at a government agency (required documents):
- An ID document upon personal visit or an electronic document from the Digital Documents service (for identification).
Result of the service delivery
A certificate from a primary health care organization, issued in the form No. 027/u, approved by the order of the Acting Minister of Health of the Republic of Kazakhstan as of October 30, 2020 No. ҚР ДСМ-175/2020 “On the approval of record-keeping forms in the field of healthcare” (registered in the State Register of Legal Acts under No. 21579), provided upon direct visit to the primary health care organization, signed by the district doctor or general practitioner, and certified with the doctor’s personal stamp and the seal of the primary health care organization.
When applying via e-Gov Portal – certificate in the form of an electronic document signed with the digital signature of the primary health care organization.
