The service can be used in the event of a change of residence (relocation), if one wishes to change the medical organization (free registration – such a change is allowed twice a year), or during the registration campaign from September 1 to November 1 (the registration takes effect from January 2026).
In accordance with paragraph 25 of the Rules for registration of individuals to healthcare organizations providing primary health care, approved by Order No. 194 of the Ministry of Health of the Republic of Kazakhstan as of November 13, 2020, the public service is provided through the e-Government portal within one working day (if the patient submits an application after working hours, on weekends, or public holidays, in accordance with the Labour Code of the Republic of Kazakhstan, the acceptance of applications and the issuance of service results are carried out on the next working day).
How to get the service online
- Log in on the Portal and click "Request Online" button.
- Fill in an application and sign it using DS (digital signature).
- In your personal account (in "History of Obtained Services" section) familiarize with a notification on application processing status, which will be sent within the specified time.
If the service delivery term is exceeded, one can contact a medical organization to which a request was sent.
How to get the service at a PHC organization (required documents):
- An ID document or an electronic document from the Digital Documents service (for identification).
- Pension certificate – for pensioners.
- Pension certificate or an extract from the medical-social examination report – for persons with disabilities.
- Court decision or another document confirming guardianship or custodianship – for the legal representatives of a child with a disability from birth, as well as for guardians, custodians, foster carers, and other substitute persons who, in accordance with the legislation of the Republic of Kazakhstan, provide care, education, upbringing, and protection of the rights and interests of the child (legal representatives).
- Minutes of the commission on the consideration of issues related to the population registration (de-registration) to primary health care in the information system “Register of Registered Population,” established by the order of the head of the regional State Healthcare Departments – for convicts serving sentences under a court verdict in places of deprivation of liberty, serving their sentences in penal colonies (at the place of serving).
- An extract from the order of the commander of the military unit (head of the institution) on the enrollment of conscripted servicemen in the personnel lists – for conscripted servicemen.
- For students at a madrasa (children under 18), an official application from their legal representatives at the administrative-territorial unit, taking into account the principle of accessibility within walking distance.
- An official request from the rector of the higher education institution to the primary health care organization and the minutes of the commission between the institution and the primary health care organization – for out-of-town students (children under 18 years old).
- A document confirming birth in a foreign country – for children born in foreign countries.
- Decision of the local executive bodies of public health administration of the regions, cities of republican significance, and the capital – for wards of baby homes, orphans, the elderly, and others.
- Persons arranging registration by proxy, including under a voluntary and/or compulsory health insurance agreement, if such an agreement exists.
Result of the service delivery
1. Notification on attachment as an electronic document signed with a digital signature.
2. Substantiated refusal.
