How to get the service online
- Sign in on the Portal and click "Request Online" button.
- Fill in an application and sign it using DS.
- In your personal account (in the "Service Obtainment History" section) familiarize with a processed certificate and submit at the place of demand.
How to get the service at a government agency (required documents):
- An ID document or an electronic document from the Digital Documents service (for identification).
- The results of clinical diagnostic studies according to the diagnosis.
Result of the service delivery
Referral to a hospital or substantiated refusal to deliver the public service.
